About
the Series Investigations is a four-part series
of videos designed for supervisors, safety officers, and other fire
and plant personnel who may - as a part of their job - be called upon
to carry out investigations of incidents in which there is property
damage and/or injury. Produced in association with the International
Fire Service Training Association, the series will help facilities
and personnel comply with federal regulations and national standards,
including NFPA 1500, Fire Department Occupational Safety &
Health Program and OSHA's Process Safety Management of Highly
Hazardous Chemicals standard. Introduction to Investigations Part One of the series, "Introduction to Investigations," outlines a systematic approach to the investigative process, showing how to interview witnesses, examine the scene, gather and maintain evidence, write reports and conduct follow-up activities. This tape provides information that is fundamental to all types of investigations, and is the foundation of the other three tapes.
Accident Investigation Part Two is "Accident Investigation." This tape explains how to carry out investigations of mechanical accidents including injuries resulting from equipment use or misuse, occupational illness, line of duty death, or accidents involving motor vehicles. The program will show how to determine conditions before the accident as well as any unusual circumstances. It also shows how to obtain professional assistance, how to file reports, and how to use findings to create a safer work environment.
HazMat Investigation Part Three of the series tells how to carry out investigations of hazardous materials leaks, spills and explosions. It teaches how to recognize the presence of hazardous materials at an incident scene, and discusses failure of containment systems. It also explains the need for scene security and personnel protective clothing and equipment. Other topics covered include use of instrumentation, obtaining assistance, and reporting requirements.
Fire Investigation The final program in the series tells how fires commonly begin and explains how fire may be expected to behave. It discusses the ways fire fighting tactics may affect the fire and destroy evidence, and provides methods for determining the point of origin and cause of a fire, including how to identify accidental fires versus arson. Other topics covered include gathering evidence and reporting.
